The office of Campus Planning, Design and Construction Management facilitates the allocation and evaluation of space throughout the University on an ongoing basis.
When employees are hired, the supervisor will fill out the space authorization form, sign it and send it via e-mail to Donna Ivancevic in Campus Planning. If approved, the form will be signed by the Space Management Office and then the form will be emailed to the proper distribution list to ensure all the services for the employee are in place. This includes office space a phone, and a computer. Once approved, the requesting department must contact Campus Services and IT directly to coordinate the details of the requested move. Please note, if a department needs new equipment, they need to email IT directly at firstname.lastname@example.org to put in the request.
When employees are relocated the supervisor will fill out the space authorization form link, sign it and send it via e-mail to Donna. If approved, the form will be signed by the Space Management Office, and the form will be emailed to the proper distribution list to ensure all services for the employee's new location are in place. This includes relocation of phone, computer and personal belongings. Please contact Campus Services to request packing supplies. Also, it is important to submit properly authorized key request forms to Campus Services at least 3 business days prior to the new employee starting or the relocation date, to ensure keys are ready for the employee on the date needed. Forms may be obtained from Campus Services in the Parking Garage.
In order to maintain accurate records of occupied space on campus, it is absolutely necessary for everyone to fill out a space authorization form. This is not just for new employees. When current employees are being relocated supervisors must also fill out the form so that the space assignments database can be updated with the correct information.
