Request an Official Transcript
A request for an academic transcript may be made in person, by mail, by fax or by e-mail with the appropriate transcript request form.
We must have a signature to release information from your academic record; therefore, we are prohibited from accepting telephone.
Benedictine University complies with all federal regulations regarding the release of student education records as established by the Family Education Rights and Privacy Act (FERPA). An academic transcript is a complete record of a student's academic achievement at Benedictine University.
There are two types of academic transcripts: official and unofficial. An official transcript is printed on transcript paper and affixed with the university seal and stamped with the Registrar's signature stamp. An unofficial transcript is printed on plain white paper with "Unofficial Transcript" written at the top. Unofficial can also be sent via e-mail whereas the official transcripts cannot.
The following information is needed to process an academic transcript request:
- Legal name (previous name(s) if applicable)
- Social security number or student ID number
- Address where the transcript is to be mailed
- Date of attendance (approx. will do)
- Daytime telephone number
- Current e-mail address
- Processing fee
Official Transcripts will NOT be processed if there are any financial or academic holds. Any request made with financial holds present will be automatically denied and the processing fee will be applied to any outstanding balance.

Request an Unofficial Transcript:
To request an unofficial transcript, please email us at EnrollmentServices@ben.edu and include the following information:
- Your first and last name while attending
- Student id number (if applicable)
- Your full birthday
- The last four digits of your social security number
- When you attended the program
- What program you attended
We will make every effort to provide you with an unofficial transcript within one to two business days of receiving the emailed request.
Release Transcripts to Family/ Friend for Pick-Up:
We take every precaution to ensure that we do not release confidential academic information to anyone without the permission of the student. If you would like to authorize a friend or family member to pick-up a transcript on your behalf without granting full FERPA authorization, you must fax us (or scan and email) a short letter authorizing the release of the transcripts.
The letter must be written by the student, include the student ID number, a copy of a valid ID (i.e. drivers licence, state issued ID, or passport), the student's signature, and it must explicitly indicate who will be picking up the transcripts. Each release is only valid for one request. Every subsequent request for a family or friend to pick-up transcripts must be accompanied with an additional letter.
