version, or 79 or above on the online version, or submission

to submit evidence of finances in order to obtain the certificate
of Eligibility (Form I-20) from the International Student Service
Office. Confidential Letter of Support

- Health
Form and Immunization:
Please be sure to complete the Health
Form in English, and mail it with your application.
Students must submit a physician's statement of a recent physical
examination in which the physician confirms that the student
is free from tuberculosis.
If you have had a TB skin test within the last year and you
sent those results, please be advised that international medical
results do not always "translate" into U.S. medical results.
Benedictine University reserves the right to reject any medical
form that is not deemed satisfactory. If this happens, you may
have to be examined and treated through the local health department;
this is a safety standard to assure that all of our students
are healthy and free of communicable diseases.
The State of Illinois Immunization Law requires a verified record
of each student's immunization history (for students taking
7.5 credit hours or more in school and born after 1956). Proof
of immunity to measles, mumps, rubella, diphtheria and tetanus
must be presented. More details can be found on the last page
of the health form.
Immunizations and TB skin tests are available through the University
Health Service office for a minimal fee.
