Network and Voicemail Request Form:

The Department Head or department supervisor completes this form online.
Fill in the requested information and email as an attachment to helpdesk@ben.edu. Department Head or Supervisor email address will be treated as the signature
In order to minimize data entry errors, handwritten forms WILL NOT BE ACCEPTED.

PeopleSoft Access Request Form:

This form must be completed online by the user’s Department Head. Access to each module of PeopleSoft requires the authorizing signature of the person listed for that section. This form should be printed, signed by each authority for the areas the user needs access to, signed by the user’s Department Head and delivered to the Helpdesk in KN 226.

Faculty Toolkit Access Form:

This form is for administrative assistants or heads of departments that need access to the class rosters based on a subject selection.
