Network and Voicemail Request Form:

The Department Head or department supervisor completes this form online.
Fill in the requested information and email as an attachment to Helpdesk@ben.edu. Department Head or Supervisor email address will be treated as the signature.
In order to minimize data entry errors, handwritten forms WILL NOT BE ACCEPTED.

PeopleSoft Access Request Form:

This form must be completed online by the user’s Department Head. Department Head's email address will be treated as the signature. Additionally, access to each module of PeopleSoft requires the authorization of the person listed for that section. Module authorization must be included in the email string prior to submitting the request to Helpdesk@ben.edu.
In order to minimize data entry errors, handwritten forms WILL NOT BE ACCEPTED.

Faculty Toolkit Access Form:

This form is for administrative assistants or heads of departments that need access to the class rosters based on a subject selection.
