LTC Rotation Assignments

Assessment of learning is ongoing throughout this Benedictine University Dietetic Internship Long Term Care (LTC) Rotation. Examples of these assessments include: pre-work, rotation responsibilities, development of an ASSURE model for inservice presentation, completion of a case study, formative and summative evaluations, data collection sheets, and a summative report of experience.

It is expected that the dietetic intern print his/her own copies of the assignment and evaluation guidelines and forms. It is required that the intern submit the completed assignments as "hard copy" paper documents to the dietetic internship director for evaluation.

Prior to the start of this LTC rotation, the dietetic intern must submit a completed Worksheet Pre-work and Required Readings Log to the Dietetic Internship Director.

Weekly responsibilities provide a suggested progression of responsibilities. The Facility Orientation Worksheet is to be completed during the first week of the onsite experience. The dietetic intern must track the nature of patient contacts using the LTC Data Collection Forms. At the end of each week the dietetic intern completes the Weekly Tally Form, followed by a 5-week Summative Tally Form. Throughout the rotation, the intern is responsible to describe significant outcomes of the learning experience and request that his/her supervisor initial the work as complete on the Work Completion Checklist.

Develop an ASSURE model to deliver as an inservice at the LTC site. The topic should be appropriate and specific to your facility; discuss your topic with your site preceptor and the Dietetic Internship Director. The written report is to include:

  1. ASSURE model with a detailed content outline,
  2. handouts,
  3. pre-test/post-test instruments,
  4. written interpretation report of pre-test/post-test results,
  5. a self-evaluation using the ASSURE Model development rubric,
  6. a self-evaluation using the ASSURE Model presentation rubric, and
  7. summary with actual Presentation Evaluation Forms completed by those in attendance of ASSURE model inservice (e.g., supervisors, dietitians, staff). Provide these forms to your audience in attendance at your inservice.

Complete a Case Study. The case study should be chosen during week 1 of the rotation. The dietetic intern shall provide all appropriate care and documentation for this resident throughout this 5-week rotation.

The dietetic intern is responsible for soliciting performance evaluations during the course of this rotation experience, as well as completing site and self-evaluations. Evaluations should be completed at the end of week 2 and week 5.

A didactic application assessment will be administered in the form of an Examination. The Dietetic Internship Director will provide the dietetic intern with a password, directions, and test data for access to the online examination.

At the end of this LTC rotation experience, the dietetic intern will submit to the Dietetic Internship Director the completed Cover Sheet for Assignments with all the assignments attached.
