Make certain to submit forms to the proper offices. Graduate School Application Submit directly to the Benedictine University Enrollment Center:   Graduate School Application Form. You can download the Benedictine University Graduate school application.   Graduate School Letter   Graduate School References (use Graduate School form). You can download the Benedictine University Graduate school reference forms.   Official transcripts (from each college/university attended)   GRE test scores   $40 Graduate School Application Fee To receive an information packet about Benedictine University and/or the program, and/or an application, please contact the Enrollment Center at 630-829-6300 or via email at admissions@ben.edu. Dietetic Internship Application As a separate packet, submit directly to the Benedictine University Nutrition Department Dietetic Internship Director:   Dietetic Internship Application Form. We utilize the computer formatted Supervised Practice Program Application and reference forms. These forms are available online from the Dietetic Educators of Practitioners practice group at http://www.depdpg.org.   Declaration of Intent/Verification Statement. Request these from your DPD program director, as they require his/her signature.   Dietetic Internship References (use correct form) from a work supervisor and at least one instructor. Submit directly to D & D Digital Systems: American Dietetic Association Dietetic Internship Preferences" form $50 fee Note: Applications will be retained for all applicants (accepted or not) for one month after the officially posted spring April match date. Application packets will NOT be returned to any appllicants.
