Make certain to submit forms to the proper offices. Submit directly to the Benedictine University Enrollment Center (application, letter, references, official transcripts, GRE/test scores, and application fee):   Download the Benedictine University Graduate school application and reference forms. To receive an information packet along with application please contact the Enrollment Center at 630-829-6300 or via email at admissions@ben.edu. As a separate packet, submit directly to the Benedictine University Nutrition Department Dietetic Internship Director (application, letter, declaration of intent/Verification Statement, references, official transcripts, and application fee): We utilize the computer formatted Supervised Practice Program Application and reference forms. These forms are available online from the Dietetic Educators of Practitioners practice group at http://www.depdpg.org. Didactic Program in Dietetics (DPD) Verification Statement or Declaration of Intent. Request these from your DPD program director, as they require his/her signature. Submit directly to D & D Digital Systems: Combined release and preference sheet.
