Instructions for filling out the Application
I have received several questions regarding the completion of
the application. The lines that hold the places between the questions
are for aesthetic purposes only. I have them on the form to show
approximately how long your answers should be. You may
take the lines off of the form in order to complete your answers.
You may type single spaced, or double spaced, paragraph style
answers depending on how much you have to say. You will not be
penalized by the length of your answer. Just be sure to answer
the questions completely.
