Forms

For your convenience, the following academic forms may be obtained
in Benedictine Central or click on the links below:
Adult
Registration Form - This registration form
is for students enrolled in the Adult program (non-cohort).
Adult
Schedule Change Form - This form is for students enrolled
in the Adult program (non-cohort). It is used to make any changes
to the student's registration schedule.
Application
for Graduation - Students applying for graduation
must complete an application. Application deadlines are published
each year in the University master calendar. The completed application
for graduation form and processing fee may be submitted to Benedictine
Central.
Approval
of Transfer Courses - When a regularly-enrolled student or
student on leave wishes to enroll in a course at another college/university,
an Approval of Transfer Courses form should be submitted to his/her
academic advisor. This form should be submitted before a course
is taken.
Change
of Student Information - The student may complete this form
to update certain demographic information, such as a mailing address
or telephone number, in the University's records. Note that some
changes, such as a name change, require the submission of supporting
legal documentation.
Change
of Academic Plan - Students who wish to change their academic
plan or sub-plan (academic major/minor), should submit this form
to their academic advisor for approval prior to submission to
Benedictine Central.
Confidential
Release Authorization - This form should be completed when
a student wishes to release personally identifiable, academic
information to a third party.
Graduate
Registration Form - This registration form is for Graduate
students (non-cohort).
Graduate
Schedule Change Form - This form is for Graduate students
(non-cohort). It is used to make any changes to the student's
registration schedule.
Petition
for External Credit - For Advanced Placement Exam,
ACT-PEP, CLEP Exam, and work/life experience credits. This form
is available in Benedictine Central and the Academic Resource
Center.
Request
for an Independent Study - Prior to registering for an independent
study course, a student must submit the Request for an Independent
Study form. This form is completed in consultation with the student's
academic advisor and requires the Department Chair's approval
before submission to the Academic Resource Center for processing.
Request
for an Incomplete - Students may request a grade of Incomplete
by initiating this form in consultation with their instructor
and approval of the Department Chair and Academic Resource Center.
Request
for an Academic Transcript ATTENTION!!!
Academic Transcripts can not be produced Friday, July 25th
through Tuesday, July 29th due to a system upgrade. Rush
processing requests must be received by 6:30pm on Wednesday,
July 23rd in order to be processed within 24 hours.
Request
to Prevent Disclosure of Directory Information - The student
may complete this form to suppress all academic and directory
information to a third party. Directory information is defined
as:
- student's name
- address
- date and place
of birth
- major field of
study
- dates of attendance
- degrees awarded
- most recent educational
institution attended
- participation in
officially recognized activities and sports
