Request for Academic Transcript

Benedictine
University complies with all federal regulations regarding the
release of student education records as established by the Family
Education Rights and Privacy Act (FERPA). An academic transcript
is a complete record of a student's academic achievement at Benedictine
University.
A request
for an academic transcript may be made in person, by mail or by
fax. We must have a signature to release information from your
academic record; therefore, we are prohibited from accepting telephone
or e-mail requests.
There are two types of academic transcripts: official and unofficial.
An official transcript is printed on transcript paper and affixed
with the university seal and stamped with the Registrar's signature
stamp. An unofficial transcript is printed on plain white paper
and stamped "Unofficial Transcript".
The following information is needed to process an academic transcript
request:
1. Legal name (previous name(s) if applicable)
2. Social security number
3. Date of birth
4. Address where the transcript is to be mailed
5. Date of attendance (approx. will do)
6. Daytime telephone number
7. Processing fee
8. Student signature

Please note: It is the university's policy to withhold the release
of an academic transcript if the student has a past-due financial
obligation owed to the university.

Transcript requests are processed via regular U.S. Mail within
7 to 10 business days. Next day delivery service via Federal Express
is available for an additional charge of $10.
