Policy:  The Office of Health Service will provide a student with a notification of illness after evaluation or will communicate an extended absence (3 days or more) to the Associate Vice -President of Student Life.  Medical documentation from your physician is required.

Additional note: This is not intended to be an excuse - only verification that student was seen in Health Service.  Instructors may have individual attendance policies. Purpose:  To provide communication to the professors, to validate excused absences, and to keep health records current.  

Extended Absences of 3 or more days - Student must get medical release before returning to school and resuming activities. The medical release should include restriction of activities, if applicable.  This information is given to the office of Student Health Services.

Additional Note: It is not the staff's responsibility to make arrangements for missed classroom assignments, materials, etc. Students are encouraged to contact instructors regarding work covered in their absence.
