Residence Life forms available for current residents:
Use this form to request any maintenance work required in your room or residence hall.
Students leaving university housing before the end of their contract must submit this form.
Students not wating to go through the proper check-out procedure near the end of the semester may fill this form. Please note that there will be a $50.00 imporper check-out fee assessed and any other room charges if applicable.
This form is required for individuals of age 21 or over to consume alcohol in Neuzil Hall.
This form must be submitted and approved before residents host a social gathering where alcohol is present.
