Tips for Locating Old Immunization Records
Immunizations are frequently needed for entry into college or other post-high school training, future employment or international travel.  At this time, there is no national data base to maintain this information.  If you are unable to obtain a copy of your immunizations , you may have to repeat vaccine doses or have blood drawn to test for immunity for certain diseases.
Some places to check for record:
Previous healthcare providers including public health departments
     Your home
Schools or other 4 year colleges
     Previous employers including the military
    Local immunization registry:  Some
states have centralized registries. For
a listing of registry contacts try the CDC
(Centers for Disease Control)
www.cdc.gov/vaccines/programs
/iis/contacts-stat-iis.htm
.  Or to find the phone number
of your local health department:
800-CDC-INFO (232-4386)
