Pre-Internship Documentation Process
CONGRATULATIONS! You’ve done the hard work of finding an internship.

You will now be required to submit several pre-internship documents (found on this page). Once all of the documents have been submitted and reviewed by a faculty member, you will be eligible to enroll in MPH 6099 Internship/Capstone for the following term.

Before submitting your documents, please notify the Internship advisors of your internship!

Meet with preceptor to complete the MPH Internship Plan and plan your schedule. You as the student should be filling out this plan; your preceptor should approve and sign it.

Complete and submit the “Affiliation Agreement” pre-screen form (insert link)

  • The Affiliation Agreement is a legal document between the University and the organization where you are interning.
  • You cannot begin your internship until it is fully signed by both sites.
  • A standard affiliation agreement has been developed by the University. Most organizations sign the standard agreement.
  • If your internship organization needs to amend the agreement, all proposed changes must be reviewed by University Compliance. The review process may take up to sixteen weeks. Please plan the start of your internship accordingly.
